Supplies and materials provided by the provider of health care over and above those usually included in a visit to his or her office or in other services rendered must be billed by report under the appropriate code set forth in the "Centers for Medicare and Medicaid Services, CMS Common Procedures Coding System (HCPCS)," as contained in the Relative Values for Physicians, as adopted pursuant to NAC 616C.145.
