Section 502.250 - Application and fee for duplicate license1. A person may apply for a duplicate license if an unexpired license previously issued has been lost, stolen or destroyed.2. An application for a duplicate license must: (a) Be submitted to the Department or a license agent on a form provided by the Department; and(b) Include an affidavit stating that the information contained in the application and any accompanying material is complete and true.3.
