Section 483.485 - Voluntary surrender of license: Contents of form; license deemed surrendered under certain circumstances1. A person who voluntarily surrenders his or her driver's license to the Department shall complete a form, approved by the Department, which contains: (a) A statement that the license is being voluntarily surrendered to the Department;(b) A statement setting forth the reasons why the license is being surrendered;(c) The person's signature; and(d) The date the license is being surrendered.2. A license shall be deemed voluntarily surrendered to the Department because of, but not limited to, the following reasons:(a) The license is held by a minor and the person who signed the application of the minor for the license dies; or(b) The communication to the Department of confidential medical reasons that render the driver either physically or mentally incapable of driving.Nev.
