Section 458.138 - Fees of Health Division1. The Division shall charge and collect nonrefundable fees, in the amount of $100 for each level of service, for the initial certification and recertification of programs and services in accordance with the application for certification or recertification provided by the Division. An operator must apply for certification or recertification for each service that will be provided by a program at each geographic location where the service will be provided.2. In addition to the fees charged pursuant to subsection 1, the Division shall charge and collect a nonrefundable fee in the amount of $50 for each geographic location that is certified with an endorsement pursuant to the criteria of the Division.Nev.
