Section 449.994 - Records required before surgery; report of surgery1. A physical examination, which must include a medical history of the patient, within the 30 days immediately preceding the date of the patient's surgery and a presurgical evaluation conducted by a physician or a podiatric physician licensed pursuant to chapter 635 of NRS, as applicable, on the day of the patient's surgery or within the 7 days immediately preceding the date of the patient's surgery must be recorded in the chart of the patient before surgery.2. A properly executed form of consent to surgery as set forth in NRS 449.710 must be placed in the medical record of the patient before surgery.3. A report must be prepared within 24 hours after surgery describing the techniques and findings of the surgery and the tissues removed or altered during the surgery. If a report is dictated, a written report must be signed by the surgeon within 7 days after the surgery.Nev. Admin. Code § 449.994
