Section 444.00803 - Client release form required; contents; maintenance on premises of establishment1. An invasive body decoration establishment must obtain a completed and signed client release form from a client before performing an invasive body decoration procedure.2. Each client release form must be maintained securely on the premises of the establishment for at least 2 years pursuant to section 44.3. A client release form must contain the following information: (a) The full legal name of the invasive body decoration operator who will perform the invasive body decoration procedure;(b) A description of the invasive body decoration procedure to be performed, including, without limitation, the type of procedure and the location on the body where the procedure is to be performed;(c) The date on which the invasive body decoration procedure will be performed;(d) An explanation that the invasive body decoration procedure should be considered permanent;(e) The full legal name of the client;(f) The date of birth of the client;(g) The mailing or physical address of the client;(h) The telephone number of the client;(i) A copy of the photographic identification of the client;(j) If the client is less than 18 years of age and is not an emancipated minor, a copy of the photographic identification and proof of custody or guardianship of the parent or legal guardian consenting to the invasive body decoration procedure;(k) If the client is an emancipated minor, a certified copy of the decree of emancipation issued to the minor pursuant to NRS 129.080 to 129.140, inclusive; and(l) The signature of the client or, if the client is less than 18 years of age and is not an emancipated minor, the parent or legal guardian consenting to the invasive body decoration procedure.Nev.
