Section 441A.230 - Duty of health care provider to report case or suspected case; content of report1. Except as otherwise provided in NAC 441A.240, a health care provider who knows of, or provides services to, a case or suspected case shall report the case or suspected case to the health authority having jurisdiction where the office of the health care provider is located. The report must be made in the manner provided in NAC 441A.225.2. The report must include: (a) The communicable disease or suspected communicable disease.(b) The name, address and, if available, telephone number of the case or suspected case.(c) The name, address and telephone number of the health care provider making the report.(d) The occupation, employer, age, sex, race and date of birth of the case or suspected case, if available.(e) The date of diagnosis of the communicable disease.(f) The date of onset of the communicable disease, if available.(g) If the case or suspected case relates to a pregnant person who has or is suspected of having syphilis, the information required by NRS 441A.150 for the case or suspected case.(h) Any other information requested by the health authority, if available.Nev.
