Section 440.360 - Signing of delayed birth certificate1. If the person of record is 18 years of age or over, the person of record must sign an affidavit provided by the State Registrar, attesting under oath to his or her belief in the truth of the statements made concerning the applicant's age, birthplace and parentage.2. If the person of record is under 18 years of age, the affidavit may be signed by the person of record or by the parents or guardian of the person of record.Nev. Admin. Code § 440.360
