Section 432A.710 - Agreement among provider, client, and parent or guardian1. A written agreement must be prepared by the client, his or her parent or guardian and the provider. The agreement must contain:(a) The rules of the program;(b) The expectations of the client and his or her family;(c) The services to be provided and cost of the services;(d) The provider's authority to give any necessary medication, first aid and medical treatment to the client;(e) Arrangements for absences, visits, vacation, mail, gifts and telephone calls; and(f) A copy of the plan for treatment of the client prepared pursuant to NAC 432A.725.2. The agreement prepared pursuant to subsection 1 must be signed by all parties and kept in the client's file. A copy of the agreement must be made available to any interested person.Nev. Admin.
