A witness subpoenaed to appear at a hearing shall be paid witness fees upon request. The witness shall receive eight dollars for each day. A request for witness fees must be made on a form furnished to the witness at the time the subpoena was received. If the witness is required to travel for an in-person hearing, the witness shall be compensated at the rate of $ 0.485 per mile for each mile actually and necessarily traveled.
