When a claimant alleges an unemployment insurance benefit check (1) has not been received or has been lost and (2) was not endorsed by claimant, a substitute check may be issued, provided:
A. It has been thirty days since the date of issuance or remailing and the check has not been presented for payment;B. The claimant files an affidavit on a form prescribed by the Commissioner stating that the check has not been received or that it has been lost and was not endorsed by the claimant; or person authorized by the claimant.C.
