Section 467-3-007 - DIABETES DIAGNOSIS AND SERVICESThis service provides treatment for diabetes mellitus.
007.01MEDICAL ELIGIBILITY CONSIDERATIONS. The only eligible diagnosis is diabetes mellitus, Type I or Type II. 007.01(A)MEDICAL ELIGIBILITY DETERMINATION. The Department determines medical eligibility for diabetes diagnosis. Medical consultant does not determine medical eligibility.007.01(B)CERTIFICATION DATE. The certification date is the date of referral, once medical and financial eligibility is met.007.02SERVICE COMPONENTS.
