Section 180-7-010 - LICENSE AMENDMENTSA licensee must apply for and receive a license amendment before:
(A) . Receiving, preparing, or using radioactive material for a type of use permitted under 180 NAC 7-007, but not authorized on the licensee's current license issued under 180 NAC 7;(B) Permitting anyone to work as an authorized user, authorized nuclear pharmacist, authorized medical physicist, or an ophthalmic physicist under the license, other than an individual who is:(i) An authorized user, who meets the requirements in 180 NAC 7-027 and 7-043(A), 7-047.01, 7-051(A), 7-052(A), 7-053(A), 7-063(A), 7-066(A), and 7-084(A);(ii) An authorized nuclear pharmacist, who meets the requirements in 180 NAC 7-024 and 7-027;(iii) An authorized medical physicist, an individual who meets the requirements in 180 NAC 7-027 and 7-023(A) and 7-023(C);(iv) Identified as an authorized user, authorized nuclear pharmacist or authorized medical physicist, or an ophthalmic physicist: (1) On an NRC or Agreement State or other equivalent permit or license recognized by the Department that authorizes the use of radioactive material in medical use or in the practice of nuclear pharmacy, respectively; or(2) On a permit by an NRC or Agreement State specific license of broad scope that authorizes the use of radioactive material in medical use or in the practice of nuclear pharmacy, respectively;(C) Changing an RSO or ARSO, other than as provided in 180 NAC 7-015.05;(D) Receiving radioactive material in excess of the amount or in a different physical or chemical form, than is authorized on the license;(E) Adding to or changing the areas of use identified in the application or on the license;(F) Changing the address or addresses of use identified in the application or on the license;(G) Changing statements, representations, and procedures that are incorporated into the license; and(H) Releasing licensed facilities for unrestricted use.180 Neb.
