Section 175-17-004 - GENERAL REQUIREMENTS

The following requirements are applicable to all licenses.

004.01EFFECTIVE DATE AND TERM OF LICENSE. ICF/ID licenses expire on March 31st of each year.

004.02LICENSE NOT TRANSFERABLE: Licenses of Intermediate Care Facilities for Individuals with Intellectual Disabilities expire on March 31st of each year.

004.03FEES. The initial and renewal licensure fees for Intermediate Care Facilities for Individuals with Intellectual Disabilities are as follows:

(A) 1 to 50 Beds	$1,550
(B) 51 to 100 Beds	$1,750
(C) 101 or more Beds	$1,950
