Section 172-131-006 - AMENDING A WHOLESALE DRUG DISTRIBUTOR APPLICATION OR LICENSEA license is issued only for the premises and persons named in the application and is not transferable or assignable. Change of ownership or change of premises terminates the license. The owner or owners must apply for a new wholesale drug distributor license.
006.01AMENDMENT. An applicant or licensee must notify the Department when there is a change in the designated representative or the information required by 21 CFR § 205.5(a). The applicant or licensee is responsible for meeting the requirements of this chapter and may amend the wholesale drug distributor application or license by submitting the required information to the Department.172 Neb.
