Section 49-2-005 - APPLICATION005.01 An application shall be submitted on the form designated by the Department and shall contain the information requested therein. The application shall be accompanied by the proper filing fee. No application shall be considered filed until the Department has deemed it substantially complete and has accepted it for filing.005.02 The original and the number of copies specified on each form shall be filed with the Department at its main office.
