Rule 36.21.703 - CONTENTS OF LICENSE(1) Each monitoring well constructor's license shall show on its face the name of the constructor, his firm name and address, the license number, and the date issued.(2) Firm name and address changes shall be submitted to the board office within 10 days after the change occurs.(3) Changes in name and address must be accompanied by the fee prescribed in ARM 36.21.415.Mont.
