Section 20 CSR 2150-2.045 - Name and Address ChangesPURPOSE: This rule outlines the requirements and procedures for notifying the board of name and address changes.
(1) Licensees must submit written notification of any address change to the board within fifteen (15) days of such occurrence.(2) Licensees whose names have changed since licensure was issued must submit a copy of the legal document verifying the name change to the board, within fifteen (15) days of such occurrence.Adopted by Missouri Register December 1, 2016/Volume 41, Number 23, effective 1/29/2017
