Section 19 CSR 73-2.120 - Duplicate LicensePURPOSE: This amendment removes the fee and the affidavit requirement.
(1) If an administrator requires a duplicate license, the administrator must submit a written request for a duplicate license to the board office, along with a written explanation as to why the original license is no longer available.
