Section 15 CSR 60-3.090 - Charitable Organization Annual Report Form

PURPOSE: This form is adopted and approved for filing as the annual report for use by charitable organizations.

Each charitable organization required by sections 407.450 through 407.478, RSMo, to file an annual report shall file an annual report on the form designated in 15 CSR 60-3.020(2). The most current version of the annual report form for use by individual professional fund-raisers may be obtained by mailing a written request to the Missouri Attorney General, Charitable Organization Registry, PO Box 899, Jefferson City, MO 65102 or in person at the Missouri Attorney General's Office, Supreme Court Building, Jefferson City, Missouri, or, if technically feasible, by downloading a copy of the form from the Attorney General's Internet website.
