Section 11 CSR 45-4.240 - Supplier's License Application and Annual Fees

PURPOSE: This rule establishes fees for all types of supplier's licenses.

(1) The one (1)-time nonrefundable application fee for a supplier's license shall be ten thousand dollars ($10,000), or a greater amount as determined by the commission. The applicant or licensee shall be assessed fees, if any, to cover the additional costs of the investigation. The executive director may waive or modify licensing fees.
(2) The annual fee for a supplier's license shall be-
(A) Five thousand dollars ($5,000);
(B) In the case of a supplier's license being issued to an applicant that is currently the holder of a temporary supplier's license, the initial annual fee shall be paid in a prorated amount of five thousand dollars ($5,000) to cover the time from the original expiration of the current temporary supplier's license until the expiration of the supplier's license; or
(C) A greater amount as determined by the commission.
(3) The annual fee for supplier's licenses is due upon issuance of the initial license and thereafter is due annually. This fee is nonrefundable and is due regardless of whether the renewal applicant obtains a renewed license.
