When an application has been approved, MDA will notify the Qualified Entity in writing. Upon receiving MDAS approval, the qualified entity may submit a request for payment form. MBE loan proceeds not disbursed by the Qualified Entity within sixty (60) days of receipt must be returned unless an extension is granted by MDA or be subject to interest penalties.
