The original and four copies of the request shall be submitted to the Commission.
The signed request must be printed or typewritten or be in legible handwriting submitted on 81/2 by 11 inch paper. The request may be in the form of a letter or may be in the form of a pleading. The request and envelope must clearly designate that a request for a declaratory opinion is sought. Oral, electronic and telephone requests are unacceptable.
