As noted in Section 108.01.2 of the Mississippi Standard Specifications for Road & Bridge Construction (as amended), for any work proposed to be performed by rental equipment, the Contractor shall, in advance of such use, notify the Resident or Project Engineer on the Department's Standard Equipment Rental Request form, more commonly known as form CAD-160. This form is maintained in the Department's Forms Manual.
