The contractor shall complete and return to the Contract Administration Division an OCR-481 Form, Disadvantaged Business Enterprise List, no later than the 10th calendar day after the opening of the bid. The contractor shall identify on the form the DBE firm to meet the project goal, the proposed specific items and percent of work to be performed, and the dollar value to be paid to the DBE firm to meet the project goal. The contractor shall also obtain the signature of the owner of the DBE firm (or the DBE designee) on the OCR-481.
