Rule 30-901-19.1 - Application for Reinstatement

Any person whose licensure or enrollment has been revoked may not apply to the Board for reinstatement of the licensure or enrollment until two years have elapsed since the final, effective date of revocation, except a person convicted of a crime of moral turpitude in which case five years shall have elapsed since the final effective date of revocation and the person's civil rights shall have been restored. In the application for reinstatement, the application should state why the licensure or enrollment should be reinstated. Applicant must include in the application evidence that the current requirements for licensure or enrollment have been met and that the applicant is in good standing with the Board.
