Any licensee who provides alternative therapy as a component of practice must, as with all other forms of practice, maintain a complete record which substantiates the care provided. Said record shall, at a minimum, include the following:
1. The medical history and physical examination(s); 2. Diagnostic, therapeutic and laboratory results; 3. Results of evaluations, consultations and referrals; 5. Discussion of risks and benefits; 6. Appropriate informed consent; 8. Medications (including date, type, dosage and quantity prescribed); 9. Instructions and agreements; and 10. Periodic reviews Records shall be current and maintained in an accessible manner, and readily available for review and inspection.
