Records for people who are provided individualized Primary Prevention or Early Intervention/Prevention Services (such as home-based individual education, parent or sibling group education, screening/assessment or Crisis Intervention Services) must be maintained with applicable forms, as required by DMH.B. Documentation of the provision of general or indirect presentations/activities on prevention and/or early intervention must include, at a minimum: 1. Topic and brief description of the presentation/activity;2. Group or a person to whom the activity was provided;4. Number of participants; and5. Name and title of presenter(s) of activity, with brief description of qualifications/experience in the topic presented.
