Section 20-2-IV - DIAGNOSTIC INJECTIONS AND PROCEDURES A.Radiofrequency Medial Branch Neurotomy/Facet Rhizotomy. This procedure may be reimbursed not to exceed three (3) contiguous spinal joint levels during the same session/procedure. If there has been improvement with a prior successful radiofrequency (RF) denervation, then a minimum time of six (6) months should elapse since prior RF denervation treatment in order to qualify for a repeat procedure. No more than two (2) RF denervations may be reimbursed in the first twelve (12) months and one (1) per year thereafter.
