Section 2-1-3-09-02-206

Applicators certified under this regulation shall keep complete and accurate records of all work performed for at least two years. Such records shall be available for examination by employees of the Bureau during reasonable business hours. Such records shall include:

1. Name and address of the person for whom the RUP is applied
2. Location of the RUP application
3. Size of the area treated
4. Crop, commodity, stored product, or site of application
5. Time and date of application
6. Brand or product name of pesticide applied
7. EPA registration number
8. Total amount of RUP per location per application
9. Name and certification number of the certified applicator
