MDHS shall ensure all serious and unusual incidents be objectively and thoroughly investigated to resolve complaints against staff, security breaches, incidents resulting from youth misconduct and other incidents when necessary. Such incidents will be conducted by Investigators from the MDHS Office of Inspector General, MDHS Human Resources, and/or other appropriate investigative office.
