Rule 18-17-5.3 - Proof of Eligibility RequirementsParents/applicants must prove eligibility requirements through the submission of documents. When possible, MDHS will access other documentary evidence in available data bases, however, proof of eligibility requirements is the applicant's responsibility.
A. Social Security Numbers - provision of social security numbers is NOT an eligibility requirement, and is strictly optional. Eligibility will not be denied due to failure of the parent to provide a Social Security Number. Social Security numbers may be used, if applicants choose to provide them, to assemble research data sets that do not identify individuals or to verify other requirements.B.
