Rule 15-5-85-4.4.1 - Death in an institution

When death occurs in a hospital or nursing home and is not a death that affects the public interest, the person in charge of that institution, or his designated representative, shall initiate the preparation of the death certificate by completing the following items:

1. Name of deceased

2. Hour and date of death

3. Place of death (hospital, institution, or physical location)
