Rule 15-19-60-6.6.2 - Procedure for Renewal of License:

The Department shall email notices, at least sixty (60) days prior to the renewal date, to the last email address registered with the Department, to the persons to whom licenses were issued or renewed during the preceding licensure period.

The licensee shall:

1. Complete the online renewal form;

3. Pay the online renewal fee;

4. Submit proof of the annual inspection and calibration of audiometric testing equipment;

5. Submit a written statement listing the worksite(s) where the licensee practices; and
