Rule 15-19-60-4.6.2 - Procedure for Renewal of License

The Department shall mail a renewal form, at least thirty (30) days prior to the renewal date, to the last address registered with the Department, to the persons to whom certification were issued or renewed during the preceding renewal period. The applicant for renewal shall:

1. Complete the renewal form.

2. Submit proof of continuing education credit as detailed in Section VII of these regulations.

3. Enclose the renewal fee.

4. File the above with the Department prior to the end of the renewal period.
