Rule 15-19-60-4.4.1 - Certification Requirement

An applicant for certification shall submit to the Department, verified by oath, written evidence in form and content satisfactory to the Department that the applicant:

1. Is licensed for the practice of funeral services as defined in Section 1-3 (F) of these rules and regulations.

2. Has successfully completed a course, approved by the Department, in eye enucleation.
