Rule 15-19-60-2.8.3 - Complaints

All complaints concerning a licensed individual, his business, or professional practice, shall be reviewed by the Department. Each complaint received shall be logged, recording at a minimum the following information:

1. name of the licensed individual;

2. name of the complaining party, if known;

3. date of complaint;

4. brief statement of complaint; and

5. disposition
