Rule 15-19-60-1.6.2 - Procedure for Renewal of License

The Department shall mail a renewal notice, at least thirty (30) days prior to the renewal date, to the last address registered with the Department, to the persons to whom licenses were issued or renewed during the preceding renewal period. The licensee shall, either in written form or electronically if within the specified renewal time:

1. complete the renewal form, and

2. submit proof of continuing education credit as detailed in Subchapter 7 of these regulations, if required, and

3. enclose the renewal fee, and

4. file the above with the Department prior to the end of the renewal period.
