Rule 15-18-77-1.5.10 - Approval

The following documentation shall be collected by the Environmentalist prior to issuance of the Final Approval to the Applicant:

1. Inspection (Form 305)

2. Affidavit (Installation)

3. Affidavit (Maintenance), for an Advanced Treatment System only

4. Fee
