Rule 15-16-1-52.18.18 - Quality Assurance ActivitiesA pharmacist shall regularly review the medication records of patients.
1. All medication orders shall be reviewed monthly by the responsible physician. Adverse drug reactions and medication errors shall be reported to the physician responsible for the patient, and shall be documented in the patient's record.2. The pharmacist in charge of dispensing medications, shall provide for monthly inspection of all storage units, including emergency boxes and emergency carts.3. A record of these inspections shall be maintained in order to verify the following:4. Disinfectants and drugs for external use are stored separately from internal and injectable medications.5.
