Rule 15-16-1-52.17.7 - Discharge Summary

A discharge summary shall be entered in the patient's record within fourteen (14) days following discharge. The discharge summary shall include but not be limited to:

1. reason for admission;

2. brief summary of treatment;

3. reason for discharge;

4. assessment of treatment plan goals and objectives; and

5. recommendations and arrangements for further treatment, including prescribed medications and aftercare.
