Treatment procedures that require special justification shall include, but not necessarily be limited to, the following:
3. The use of electroconvulsive therapy and other forms of convulsive therapy;4. The performance of psychosurgery of other surgical procedures for the intervention in, or alteration of, a mental, emotional, or behavioral disorder;5. The use of behavior modification procedures that use painful stimuli;6. The use of unusual medications and investigational and experimental drugs;7. The prescribing and administering of drugs for maintenance use that have abuse potential (usually considered to be Schedule II drugs), and drugs that are known to involve substantial risk or to be associated with undesirable side effects; and8.
