Rule 15-16-1-45.13.3 - Admission Agreement

Prior to or at the time of admission, the administrator and the resident or the resident's responsible party shall execute in writing a financial agreement. This agreement shall be prepared and signed in two or more copies, one copy given to the resident or his sponsor, and one copy placed on file in the license facility.As a minimum this agreement shall contain:

1. Basic charges agreed upon (room, board, laundry, nursing, and/or personal care).

2. Period to be covered in the charges.

3. Services for which special charges are made.

4. Agreement regarding refund for any payments made in advance.
