Rule 15-16-1-43.5.6 - Mississippi State Department of Health shall be notified, in writing, of any of the following within 30 days prior the occurrence
1. Address/location,
2. Facility name,
3. Phone number;
4. Hours of operation/24-hour contact procedure,
5. Change in address or phone number,
6. Administrator,
7. Director of nursing, and
8. Cessation of business.
