Rule 15-16-1-41.29.1

Adequate medical records on each patient shall be kept. The emergency medical record shall contain:

1. Patient identification.

2. History of disease or injury.

3. Physical findings.

4. Laboratory and x-ray reports, if any.

5. Diagnosis.

6. Record of treatment.

7. Disposition of the case.

8. Signature of a physician.
