Rule 15-16-1-4.12.4 - The following discharge information shall be recorded in the client's record

The following discharge information shall be recorded in the client's record:

1. Date of discharge;
2. Transfer facility;
3. Reason(s) for discharge; and
4. Condition upon discharge.
