The hospice shall have a governing body that assumes full legal responsibility for determining, implementing, and monitoring policies governing the hospice's total operation. No contracts/arrangements or other agreements may limit or diminish the responsibility of the governing body. The governing body shall:
1. Designate an individual who is responsible for the day-to-day management of the hospice program;2. Ensure that all services provided are consistent with accepted standards of practice;3. Develop and approve policies and procedures which define and describe the scope of services offered;4. Review policies and procedures at least annually revise them as necessary;5. Maintain an organizational chart that delineates lines of authority and responsibility for all hospice personnel;6. Meet no less frequently than quarterly; and7.
