Rule 15-12-31-1.14.1 - Complaints:

All complaints concerning certified personnel or licensed service shall be reviewed by the BEMS. Each complaint shall be logged, recording at minimum the following information:

1. Date of Complaint Submission.

2. Name of the complainant with contact information (if known).

3. Nature of the complaint.

4. Date and Time of Event.

5. Brief statement of the complaint

6. Disposition
