Rule 1-5-6.2 - Oaths and affirmationsA. The individual taking the oath or affirmation shall appear personally before the notary public.B. The notary public shall have personal knowledge or satisfactory evidence of the identity of the individual taking the oath or affirmation.C. An oath or affirmation may be verbal or in writing. If in writing, the oath or affirmation shall be signed in the presence of the notary public.D. In administering an oath or affirmation, the notary public shall require the individual taking the oath or affirmation to voluntarily swear or affirm, under penalty of perjury, that the statements contained in the oath or affirmation are true, or that the individual will perform an act or duty faithfully and truthfully.
