Part 9555.6115 - LICENSE APPLICATION PROCESS A license to operate an adult foster home must be obtained from the department under items A to D.
A. Application for a license must be made on the application form issued by the department and must be made in the county where the adult foster home is located.B. The applicant must be the individual or the authorized representative of the partnership, corporation, or government unit that will be the operator of the adult foster home.C. Separate licenses are required for residences located at separate addresses, even if the residences are to be operated by the same operator.D. An application for licensure is complete when the commissioner determines that the department forms and documentation needed for licensure, the inspection, zoning, evaluation, and investigative reports, documentation and information required to verify compliance with parts 9555.5105 and 9555.6105 to 9555.6265 have been submitted by the applicant. The commissioner shall stamp and date a signed and completed department application form on the date of receipt.Statutory Authority: MS s 245A.09
